Referral pathway

Clear steps from initial enquiry to placement review

Maridi Care welcomes referrals and enquiries from local authorities, NHS teams, social workers, hospital discharge teams, housing and homelessness services and other professional partners.

Our referral process

Six steps

  1. Step 1: Referral received

Send us information about the individual and the proposed placement.

  2. Step 2: Information reviewed

We review the available assessments and supporting information.

  3. Step 3: Assessment & risk review

We consider the person's needs, risks, preferences and compatibility with the shared environment.

  4. Step 4: Support proposal issued

Where appropriate, we provide a proposed support package and associated fees.

  5. Step 5: Placement agreement

The placement arrangements, funding and responsibilities are agreed.

  6. Step 6: Admission & review
