Support designed around you

There is no single support package that works for everyone. Support is structured, but it adapts to individual needs.

Before a placement begins

We seek to understand the person’s needs, strengths, risks, goals and preferences. This information is used to develop an individual support plan, which is reviewed as circumstances change.

What support may include

Structured support that adapts to individual needs

Initial assessment
Before a placement begins we seek to understand the person's needs, strengths, risks, goals and preferences.
Personalised support planning
Working with each resident to identify goals and the support needed to achieve them.
Key worker support
A named key worker and regular check-ins, so residents have opportunities to talk about progress, challenges and what matters to them.
Recovery & emotional wellbeing
Providing encouragement, reassurance and a stable environment that supports recovery.
Daily living skills
Support may include routines, cooking, shopping, budgeting, household responsibilities and other independent living skills.
Community access
Helping residents access appointments, local facilities, education, employment opportunities and community activities.
Health & wellbeing
Signposting and helping residents access appropriate healthcare and specialist services.
Medication prompting
Where appropriate and agreed, we can remind residents to take medication they manage themselves.
Multi-agency working
We work with social workers, NHS teams, families and other professionals where appropriate and with the necessary consent.

Support is focused on helping residents stabilise, regain confidence and progress towards greater independence.

Additional staffing

Enhanced & one-to-one support

Where an assessment identifies additional needs, we can discuss enhanced staffing or one-to-one support with the referring professional or commissioner.

Enhanced support and 1:1 arrangements can be proposed where risks or needs require additional staffing. Any additional support is individually planned and reviewed.
